PLEASE NOTE - All foreign national security clearance forms must be received
and processed three months prior to the beginning of the cruise. Please send
these forms as soon as possible.

Each foreign national (FN) guest must complete the following Foreign National
Vistor/Guest Coversheet Report form (Office of Security (OSY) Eastern Region
FN Submission MAPS form) and email the completed form to Betty Huss
(Betty.Huss@noaa.gov). The purpose of visit, sponsor information, and duty
dates have already been entered on this form. All other fields, must be
filled in by the FN.

Each FN must sign the following Appendix C form and either mail the form
or send the signed form in pdf format as an email attachment to Betty Huss
(Betty.Huss@noaa.gov).
