PLEASE NOTE - All foreign national security clearance forms must be processed
and cleared at least one month prior to the beginning of the cruise.

Each foreign national (FN) must complete the following Office of Security (OSY)
Eastern Region FN Submission MAPS form and fax the completed form to Betty Huss.
The fax number is 305-361-4447.  The sponsor information, visitor's proposed working
location, length of stay and duty dates have already been entered on this form.  All the
other fields must be filled in by the FN.  Please notify Betty Huss (Betty.Huss@noaa.gov)
when the fax has been sent.

        Office of Security (OSY) Eastern Region Form

Each FN must sign the following Appendix C form and send the signed form in pdf format
as an email attachment to Betty Huss (Betty.Huss@noaa.gov).
