PLEASE NOTE - All foreign national security clearance forms must be received
and processed three months prior to the beginning of the cruise. Please send
these forms as soon as possible.

Each foreign national (FN) guest must complete the following Foreign National
Vistor/Guest Coversheet Report form and fax the completed form to Betty Huss
(Fax number 305-361-4392). On the fax coversheet, please put IO7N. Please send me
an email (Betty.Huss@noaa.gov) when the fax is sent. The purpose of visit and
duty dates have already been entered on this form. All other fields except the
Sponsor must be filled in by the FN.

        Foreign National Visitor/Guest Coversheet Report Form

Each FN must sign the following Appendix C form and either mail the form
or send the signed form in pdf format as an email attachment to Betty Huss
(Betty.Huss@noaa.gov).

        Appendix C

Each FN must fill out the following sections of Part A of the Endorsement Supplement
Form: Home Country, Non-NOAA Afflication, Title, Organization, Education, and the
Qualifications and Expertise elements in the Justification section. Please send the
Endorsement Supplement form to Betty Huss (Betty.Huss@noaa.gov).
